Evolve Healthcare – Ageing Support are searching for an experienced RN – Clinical Care Coordinator to work with our Residential Aged Care client in Hervey Bay!
The Care Coordinator is responsible for providing leadership, support and guidance to clinical and care staff in the provision of quality care based on assessed residents; needs that enhances and promotes quality of life, ensuring that care is consistent with the mission of our client and current best practice. Support the Residential Services Manager / Care Manager to provide effective leadership and management of clinical and care staff to facilitate quality care based on assessed residents’ needs that enhances and promotes quality of life.

• Bachelor of Nursing + APHRA Registration 
• Full immunisation history

• First Aid & CPR Certificates
