Evolve Ageing Support are searching for a highly skilled Nurse Practitioner – Residential Aged Care based in Cessnock NSW.
This is a rare opportunity to cement yourself within a notarised NFP in the region, working alongside the Group Services Manager providing excellence in care for for 150 Aged Care Residents, their families and support networks.
The Role
The Nurse Practitioner, who is a registered nurse educated to function autonomously and collaboratively in an advanced clinical role, will provide expert assessment and management of residents/consumers using nursing knowledge and skills and may include but is not limited to the direct referral of residents/consumers to other health professionals, prescribing medications and ordering diagnostics investigations.
- Provision of expert clinical care and decision making that leads to person centered, end of life experience, for residents/consumers with advanced life limiting chronic disease including dementia
- Provision of expert acute clinical care to identify and manage acute clinical conditions affecting residents/consumers.
- Providing advanced clinical skills and assessment/intervention and consultation.
- Developing a ‘shared care’ partnership with Residential Care Managers, GP’s and other health professionals to ensure a collaborative and consensus building approach to end of life care planning, supporting self-determination and promoting person and relationship centered care.
- Contribute to broader goals of hospital avoidance and improvement of the resident/consumer journey through person-centered care planning.
- The delivery of pharmacological and non-pharmacological management strategies.
- The assessment of side effect management for the resident/consumer receiving interventions.
- Provision of information with residents/consumers, carers and health practitioners relating to the diagnosis, treatment options and treatment methods;
- The development of clinical guidelines associated with the speciality of aged care;
- Provision of clinical supervision and mentoring across the organisation
The Offer
- Annual salary negotiable based on experience $143k – $147k
- Salary Packaging up to $16,500 pre tax + Superannuation
- 4 weeks relocation support (temporary accommodation)
- $1000 relocation bonus (flights, hire car etc)
- Full Time role, 3 facilities all in close proximity with free car parking
Highly Desirable – New Grad NP’s will be considered
- Registered Nurse with post graduate qualifications as a Nurse Practitioner and eligible to apply for endorsement with the Australian Health Practitioners Regulation Authority (APHRA).
- Demonstrated evidence of recent Professional Development in the areas of Palliative and Aged Care with extensive skills and knowledge in advance care planning, the palliative approach in aged care and end of life care for older people facing life limiting illness.
- Minimum of 5 years full time post registration experience, with a minimum of three years full time within the aged or community care environment.
- Extensive clinical experience working within residential and community aged care with residents/consumers living with advanced chronic disease and frailty.
- Proven record of experience in project management and reporting against a Commonwealth evaluation strategy including experience in the development and review of policy, clinical guidelines and clinical pathways/frameworks.
- Nurse Practitioner Standards of Practice (Nursing and Midwifery Board of Australia).
- Demonstrated knowledge and understanding of Commonwealth and State legislation, and Policy, governing the provision of aged care quality services.
- Demonstrated knowledge and understanding of palliative care services and other support services available to older people; and their families, facing life limiting illness.
- Provision of evidence of the development of resources to support education and relevant information for residents/consumers, carers and professionals.
